In 2003 DAM started a new department: The Home-Based Care. About twenty caregivers were taking care of people with chronic diseases such as HIV and AIDS, Tuberculosis, Hypertension and Diabetes. The caregivers assisted the patients in their daily life by bed-bathing those who were confined to bed, cleaning their rooms, offering counselling, doing physio and collecting their medication.
In 2013 the government transferred all the home-based caregivers to the clinics. DAM now has a partnership with the Soweto Home-Based Care Givers, who are still doing home visits independently from the clinics.
